Fees for Services - $335.00 per day ($235/half day) plus $7.00 per patient for each patient treated over 40 patients and $7.00 for each exam performed after the second exam for the day, for each day of service I provide. Travel expenses include $45 per day if one way travel is greater than 45 minutes. An additional $10.00 for each 15 minutes over one (1) hour will be added. Total fee for the day would be $335.00 for the day ($235/half day), payable at end of the last day of services provided. If more than 40 patient’s are treated in a given day and/or each exam after the first two for the day, the total amount of patients treated above 40 are multiplied by $7.00 and exams starting with the third one for the day, multiplied by $7.00, will be added to the base fee of $335.00 ( or $235/half day). If more than 45 minutes of travel one way is required each day, then an additional $45.00 will be added to the fee for that day’s service and an additional $10.00 per each 15 minutes above 1 hour of travel for one way only. Add any additional time fee for services performed outside of normal clinic hours.

If services necessitate that overnight stay is required for vacation relief, then retaining Doctor may make arrangements for Dr. Bruno’s overnight stay subject to Dr. Bruno’s prior approval.
